Task 8.2.7 prepares upcoming Survivorship Teaming Activities in Brussels and Luxembourg

This month, Task 8.2.7 continued developing the Survivorship Hub and the broader survivorship implementation framework. The Patient Navigation Malta Report and Patient Navigation Implementation Framework were shared with partners for review, while a survey draft to identify potential CCCs for patient navigation implementation is being prepared for dissemination.

The Survivorship Hub is being refined as a living model, designed to map survivorship care across the patient pathway, from diagnosis through long-term follow-up and balance of life. The task also focused on preparing the next teaming activities.

The Brussels Teaming Activity, planned for 17–18 September 2026, is in active preparation and will focus on screening tools for survivorship care, needs assessment and stratification. Partners were encouraged to register and bring practical tools, examples and proposals to support the working sessions.

In parallel, the Luxembourg Teaming Activity, planned for Q1 2027, is under development and will focus on prehabilitation and rehabilitation, continuing the task’s approach of linking each teaming activity to a concrete, trackable deliverable.
